Required Paperwork:

• Youth Roster Form
• Youth Waiver - Every player must submit this form (must have original form, no scanned copies, no faxed copies)
• Proof of Age - Copy of Player Cards (USYS or US Club Soccer), copy of Birth Certificate or Passport for every player.

• Two registration nights will be held for all teams that have already submitted their team’s application along with payment.  During this time, teams should bring the team roster and  waivers and proof of age for all players.
